In order to efficiently process your claims please review the FAQs below. Please contact us at 800-482-8770 with questions.

1. How do I submit my claims electronically? We utilize Optum’s Clearinghouse for processing claims electronically. Our Clearinghouse number is 96277. Please take advantage of submitting your claims electronically. By doing so your claim processing time can be reduced significantly.

2. Where do I mail my claims? If you choose to mail your claims to us, please follow these instructions.

                  A. Mailing Address:

                  B. DO NOT STAPLE OR TAPE your claims within the envelope, this will delay processing.

                  C. Please include the OptiMedHealth Member Number, NOT the MAJOR Medical Member number when submitting GAP or Secondary Claims. Processing will be delayed if you submit incorrect member numbers on your claim form.

                  D. If you are submitting a multiple-page HCFA or UB-04 claim please indicate the page numbers on the forms, ex. Page 1 of 2.

3. How can I check the status of a claim I submitted? Once you have submitted your claims you can check online to see the status of the claim.   Please allow 3-10 days for your claim to be received and processed before being available to view online. For mailed claims, please allow 10-15 days for us to receive your claim in the mail.

4. I received a rejection letter regarding a claim submitted. What can I do to get my claim processed? This means your claim has been rejected, NOT DENIED, as we need more information in order to process the claim. Please resubmit the claim with the information requested in the rejection letter.

5. We do not provide a fax number for claims submission. However if you would like to submit a claim via secure email you can submit the claim to claims@optimedhealth.com.

6. You can submit questions, appeals, or any other materials to us via customercare@optimedhealth.com.
